29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a) West Wall, Shop Area, The Hudson Company, 2290 Route 199, Pine Plains, NY: On or about 2/21/2014; Access to the designated emergency exit door was blocked by a removable table and bundles of lumber, exposing employees to slip and fall hazards while trying to access the exit.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Shop, On or about 2/21/2014; Lockout Procedures, employee training, and periodic inspections were not conducted or documented for equipment including, but not limited to; Oliver Straitoplane machine, the Wood-Mizer molder and the G0504 16" Dual Conveyor Re-Saw, exposing employees to amputation hazards.

This exposed employees to struck-by and crushing hazards.
