29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    Establishment:  Where the employer had established a written respiratory protection program, employee training on this program and key elements were not satisfied in the following instances:      a.     Section III of the program did not list the specific job duties that require the use of respirators.  b.     Section V of the program did not indicate that a hazard assessment for respirator hazards had been conducted (Most evaluations are done through air monitoring).  c.     Attachment B was not properly completed to indicate the specific type(s) of respirators the employees are using for each job task that require its use.      ABATEMENT DOCUMENTATION REQUIRED
