29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: On or about February 20, 2014, and at times prior thereto, an employee was using an iron worker with two instances of not having the point of operation guarded: 1) The shear on the back of the iron worker was equipped with a guard, but was not used during the operation of the shear. 2) The punch press at the front of the iron worker did not have a guard at the point of operation. The employee was exposed to amputation hazards.
