29 CFR 1910.178(h)(2): Where general lighting was less than 2 lumens per square foot, auxiliary directional lighting was not provided on the truck. a) Dock #7, 1104 North Park Drive, Hazleton, PA - An employee used a Raymond Sit-Down Counterbalanced Truck Model 445-C40TT (Serial No. 445-13-11351) with a broken headlight to enter and remove wooden pallets from an unlit commercial motor vehicle towed trailer. The employee was exposed to struck-against and fall from elevation to lower surface hazards due to a lack of lighting at or above 2 lumens per square foot, as observed on or before February 21, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

The employee was exposed to struck-against and fall from elevation to lower surface hazards due to a defective forklift, as observed on or before February 21, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
