29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:  (a) Construction Site: On or about February 20, 2014, and at times prior, employees were exposed to an exterior fall of approximately twenty-five feet to the ground below while wearing personal fall arrest system with lanyard secured to the parapet frame.  (b) Construction Site: On or about February 20, 2014, and at times prior, employees were exposed to an interior fall of approximately ten feet four inches to the floor below while wearing personal fall arrest system with lanyard secured to the parapet frame.
