29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    (a) Front side, new residential construction located at 842 Nottingham Drive, Smyrna, GA 30082: The employer allowed employees to work on and from a pump jack scaffold system with the supporting poles not resting on a firm foundation exposing employees to fall hazards.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    a) Front side, new residential construction located at 842 Nottingham Drive, Smyrna, GA 30082: The employer failed to secure the bottom of the pump jack scaffold support poles to prevent accidental movement or collapse of the system during use. The support poles were not plumb and square which compromises the strength of the scaffold system. Employees were exposed to falls hazards from potential collapse of the system as supporting bracing or equivalent were not used.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a) Front side, new residential construction located at 842 Nottingham Drive, Smyrna, GA 30082: Employees were exposed to falls of approximately 14-feet, 10-inches while installing Hardie-Shake Siding from a pump jack scaffold platform which was not provided with a fall protection system. The employer failed to provide guardrails along all open sides or provide personal fall arrest systems.
