29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Behind Erema #4; The tongue guard of Skill 3380 pedestal grinder was not adjusted to within one-fourth inch of the wheel periphery. The grinder is used to clean plastic off blades taken out of the compactors.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) Behind Erema #4: A panel box supplied with 440 volts of electricity was left with the door open such that live parts were not guarded from accidental contact. For approximately three months the interior of the box was accessed daily to manually turn water pumps on and/or off.
