29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks. a) Building A1; Laminating Department; Machine #1 - Machine operators were exposed to ingoing nip points and rotating parts while wiping down the rotating rollers in the glass/mat machine, on or about February 20, 2014. b) Building A1; Laminating Department; Machine #2 - Machine operators were exposed to ingoing nip points and rotating parts while wiping down the rotating rollers in the glass/mat laminating machine, on or about February 20, 2014. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
