29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment: a/ Premier Die Casting/Rahway NJ Site: Foundry Charging Well MPH Area at the rear of the building. The hazard assessment provided by the employer for personal protective equipment did not include all of the protectors needed to prevent employee exposure to molten metal in the event of a splash during the furnace charging operation. On/or about 02/11/2014 an employee's neck, arm and leg was not protected when splashed by molten metal during a furnace charging operation. Observed on/or about 3/26/2014.

29 CFR 1910.1200(h)(2): Employees were not provided information and training on the new 2012 Hazard Communication Global Harmonizing System: a/ Premier Die Casting/Rahway NJ Site: Plant. Training was not provided to employees on the new chemical Data Sheets and Labeling - effective December 1, 2013. Safety Data Sheets (formerly Material Safety Data Sheets) are maintained by the employer for chemicals such as but not limited to TRIM SOL water miscible cutting and grinding fluid concentrate. Observed on/or about 03/19/2014.
