A) 1177 Rahway Avenue Avenel, NJ 07001 Tool casting room In the tool casting room, the employer did not ensure hooks used as lifting devices had safety latches covering the throat openings, exposing employees to struck-by hazards in the event of equipment failure; The double picking hooks used in the tool casting room were not equipped with safety latches.
