29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.    Location: 724 5th Avenue Brooklyn, NY.     On or about 01/30/13      a) Four employees were performing demolition from a  tubular welded frame scaffold that was not fully planked.  The employees were exposed to a potential fall hazard of up to 30 feet to the ground below.

29 CFR 1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.       Location: 724 5th Avenue Brooklyn, NY.     On or about 01/30/13    a) Four employees were working from a tubular welded frame scaffold that did not have means of access. Employees climbed frames and crossbraces to get to work platform.

Location: 724 5th Avenue Brooklyn, NY.   On or about 01/30/13  a) Employees were not trained to recognize fall hazards. Four employees were working by the edge of anew floor without any fall protection while exposed to a potential fall of approximately 20 feet to ground below.
