Location: 724 5th Avenue Brooklyn, NY.   On or about: 09/23/14   a) Employees used a power strip to energize a Makita angle grinder and Circular saw. The power strip was not designed to be used on a construction worksite.

29 CFR 1926.404(b)(1): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.  Location: 724 5th Avenue Brooklyn, NY.  On or about: 09/23/14  a) Employees used a Makita Angle Grinder witch was not protected with GFCI protection or an assured equipment grounding program.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    Location: 724 5th Avenue Brooklyn, NY. / First Floor.  On or about: 09/23/14  a) Employees worked next to unprotected floor holes located along the first floor.  Employees exposed to a fall of approximately 15 ft. from the 1st floor to the basement.    Ateweek, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 959848, citation number 1, item number 4 (a) and was affirmed as a final order on 03/27/14, with respect to a workplace located at 724 5th Avenue Brooklyn, NY 11232.
