29 CFR 1910.125(c)(1)(i): Dip tank was not equipped with an automatic closing cover meeting the requirements of paragraph (f)(3) of this section:     In the Machine Shop, On or about February 25, 2014, employees were exposed to burn hazards when using a Dyna Clean parts washer and degreaser which had a cover, but had a wire being used in place of a fusible link, while employees were using the dip tank for cleaning parts.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   a. In the Machine Shop, On or about February 25, 2014, employees were exposed to damage of the nervous system when using the KWIK DRI 66 parts cleaner solution containing mineral spirits which is a petroleum distillate and is labeled toxic to the nervous system, while washing their hands with the solution.   b. In the Machine Shop, On or about February 25, 2014, employees were exposed to health hazards when a container was mislabeled containing used oil instead of Polyphase 663, while employees dump used oil into the container.
