On or about February 25, 2014 the employer stored four cylinders of compressed carbon dioxide gas in the warehouse area at the carbon dioxide filling station where one cylinder did not have a valve cap installed and the cylinders were not provided protection from cut, abrasion, or impact by heavy moving objects.
