29 CFR 1960.8(a): 29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: (a) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to the hazard of being infected with Mycobacterium Tuberculosis through unprotected contact with patients who was/were infectious or suspected to be infectious with tuberculosis in that a TB isolation room (Room No. 8-61) located on the 8th-floor (Inpatient Ward) of the hospital was not being maintained under negative pressure as determined through smoke trail tests. Feasible and useful abatement methods for reducing this hazard, as recommended by the CDC, include, but are not limited to making appropriate adjustments to the ventilation system to control the direction of airflow between the TB isolation room and adjacent areas to prevent contaminated air inside the room from escaping into other areas of the facility. ABATEMENT CERTIFICATION IS REQUIIRED

Located at Moncrief Army Community Hospital, Fort Jackson, SC: On or about 2/11/14, the employer did not record the following workplace injury correctly on the OSHA 300 Log for calendar year 2013: (a) On or about 10/21/13, a registered nurse sustained a strained lower back while working in Same Day Surgery, resulting in 14 restricted workdays. The employer properly classified the case on the Log, but did not provide an entry of the number of restricted workdays the employee experienced.
