29 CFR 1904.32(b)(1): The employer did not extensively review the OSHA 300A Annual Summary to ensure that entries are correct and complete:  (a) Installation Safety Office, Fort Huachuca, AZ 85670:  The employer did not review the OSHA 300A Annual Summary from Calendar Year 2013 to ensure that the entries accurately reflected the cases entered on the OSHA 300 Log for 2013, in the following cases:  (1) 26 cases of the total 35 cases did not have entries in column (F) of the OSHA 300 Log which described the injury or illness, parts of the body, and the object or substance that directly impaired the exposed employee in determining the recordability factor of the specific cases;  (2) 11 cases of the total 35 cases were not identified as to "Other Medical" (column J on the on the OSHA 300 Log), which incorrectly resulted in zero cases (0) total for the OSHA 300A Annual Summary in colum (J) for 2013;  (3) 1 case was not identified in column (H) of the OSHA 300 Log, which resulted in 19 "Days Aways" cases total on the OSHA 300A, instead of the correct number of 20 cases;  (4) 1 case (#100251943) was mis-classified as an illness (column M4) on the 300 Log rather than correctly as an injury (Column M1), which resulted as incorrect accounting of 34 injuries and 1 illness, rather than 35 injuries for 2013 on the OSHA 300A Annual Summary.
