Employees were exposed to electrical shock hazards while plugging into a makeshift electrical outlet which was made of an electrical junction box, 14/2 Romex cord, cover plate and electrical tape. The electrical components used were not used as intended by the manufacturer as they were used outdoors and not fixed to a support within a structure.

Employee was exposed to falls of approximately 9-feet, 8-inches while installing roof rafters from the top plate located over the garage with no fall protection system in place.  Rudolfo Arriaga was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 954275, citation number 1, item number 2, and was affirmed as a final order on 02/28/2014, with respect to a workplace located at 1560 Roswell Street, Smyrna, GA 30080.
