29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about February 28, 2014, and at times prior thereto, employees added corrosive liquid Baracor 95 to the suction tank on the mud tanks through an open floor hatch while wearing safety glasses instead of chemical goggles or face shield, exposing employees to the hazards of corrosive chemical splashes.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about February 28, 2014, and at times prior thereto, on the mud tanks, where employees were adding corrosive Baracor 95 and Caustic Soda, the eyewash station was located on the far side of the sink so that employees would have to walk around obstacles such as pipes and hoses to reach it, exposing employees to the hazards of corrosive chemical splashes.
