29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:    On or about February 28, 2014 the cover of the Square D breaker panel providing power to the Rig manager's trailer had a broken door hinge.  The breaker panel was located on the side of Rig manager's trailer, found on the Makers 1-H well site.

29 CFR 1910.303(b)(7):  Electrical equipment was not installed in a neat and workmanlike manner:    On or about February 28, 2014 a 75 KVA transformer had 7 screws missing including all the screws at the bottom of the front cover that kept the cover from falling down on live connections.  One half of the Weather guard was also missing.  The transformer was providing power for the Rig manager's trailer and other trailers, located on the edge of the drilling site, found on the Makers 1-H well.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures were not weatherproof in wet locations:  On or about February 28, 2014 the 75 KVA transformer providing power for the rig trailers had one side of the "weather shield" missing.  This condition created an electrical hazard to employee in the event rain water entered the transformer.  The transformer was located on the outside of the rig manager's trailer, found on the Makers 1-H well site.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about February 28, 2014, the power cord energizing the changing trailer did not have an effective strain relief device, the cord was located on the side of the transformer, found on the outside of the rig manager's trailer.
