Employees used a Ridgid Wet/Dry shop vacuum cleaner for cleaning up combustible wood dust on and around the Ridgid table saw.     B. A four-plug outlet which energized the Ridgid Table Saw was installed on the floor next to the table saw and saw dust had accumulated inside the outlet box where the live wiring was housed.

Machine Specific Procedures were not in place for multi-energy source equipment like pumps.  B. Training was not provided after an employee had recieved initial training upon hire.
