29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about March 4, 2014, an employee using a Hitachi pneumatic air nail gun was not wearing safety glasses, exposing himself to being hit by nails or flying deris.   b. On or about March 4, 2014, an employee using a compact circular handsaw was not wearing safety glasses, exposing himself to being hit by flying debris.

On or about March 4, 2014, a Rolair air compressor was not guarded, exposing employees to struck-by or caught-in hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about March 4, 2014, on a 5:12 pitch roof, five employees performing rooing operations were not protected from an 8-foot 8-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.    b. On or about March 4, 2014, on a 5:12 pitch roof, employees performing roofing operations were not protected from stepping or falling into a hole in the roof, which was an 8-foot interior fall hazard by use of a personal fall arrest system, covers or guardrails erected around the hole.
