29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: On or about March 5, 2014, at a jobsite located at 606 Wyoming Dr. Murphy, TX 75094; employees were working from the second tier of a tubular welded scaffold platform that was approximately 10 inches wide and were exposed to a fall hazard of approximately 12 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about March 5, 2014, at a jobsite located at 606 Wyoming Dr. Murphy, TX 75094; employees climbed 12-feet on crossbraces from the lower surface level to access the second tier of a tubular welded scaffold. The employer did not provide a ladder or other means of direct access to the second tier of the scaffold and employees were exposed to the hazards of falling to the ground below.
