29 CFR 1926.307(f)(1): Gears shall be guarded in accordance with one of the following methods: by a complete enclosure; or by a standard guard as described in paragraph (o) of this section; or by a band covering the face of gear and having flanges extended inward beyond the root of the teeth on the exposed side or sides. (a) The employer exposed his employees to caught-in hazards when a masonry mixer was found being used with open and rotating gears.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) The employer did not ensure that each employee working off a scaffold approximately twelve (12) feet or more above the ground level below were protected from fall hazards.
