29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Right-side Polishing Area, on or about 3/5/2014: A Dayton Industrial Motor with unguarded rotating chuck was used by employees to polish and buff metal rods. b) Left-side Polishing Area, on or about 3/5/2014: Le Blonde Lathe with unguarded rotating chuck was used by employees to polish and buff metal rods. c) Back-side Cutting Area, on or about 4/3/2014: Two Jet Milling Machines were not provided with a guard over the rotating chuck. d) Back-side Cutting Area, on or about 3/5/2014: Jet Band Saw with unused portion of the blade unguarded was used by employees for cutting metal. e) Front-side CNC Area, on or about 3/5/2014: Jet Drill Press with unguarded rotating chuck was used by employees to drill and tap parts. f) Front-side CNC Area, on or about 3/5/2014: Jet Lathe with unguarded rotating chuck was used by employees for tooling. g) Front-side CNC Area, on or about 4/3/2014: Shop Fox Drill Press with unguarded rotating chuck was used by employees for chamfer parts.
