29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    (a)  Back of Building - On or about February 28, 2014 employees were exposed to a fall hazard due to a scaffold not being fully decked or planked. One deck was not installed on the scaffold resulting in a 16.5 inch gap where the employee could fall 11.6 feet onto concrete.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.     (a)  Back of Building - On or about February 28, 2014 employees were exposed to a fall hazard while standing on a scaffold 11.6 feet above concrete with no guardrail system.

29 CFR 1926.452(w)(1): Scaffolds were not braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections were not secured.     (a)  Back of Building - On or about February 28, 2014 employees were exposed to a fall hazard while working on a scaffold that was missing cross-braces on the inside. Employees were exposed to falling 11.6 feet to a lower level.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces shall not be used as a means of access.  (a)  Back of Building - On or about February 28, 2014 employees were exposed to a fall hazard due to scaffold having a ladder that did not reach within 2 feet of the ground.  The access point was 78 inches above concrete.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.   (a)  Back of Building - On or about February 28, 2014 employees were exposed to several fall hazards due to the fact the scaffold had a damaged platform and boards. Scaffold was not inspected for visible damage prior to use.
