29 CFR 1910.147(c)(4)(ii)(C): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them:   a.   At the workplace, the employer's written Lockout/Tagout Procedure, Section VII Removal of Lockout Devices, did not include the specific procedures for lockout/tagout device removal when the authorized employee who applied the lockout/tagout device was not able to remove it including, but not limited to, the following:  Verification by the employer that the authorized employee who applied the device is not at the facility; Making all reasonable efforts to contact the authorized employee to inform him/her that his/her lockout or tagout device has been removed; and Ensuring that the authorized employee has this knowledge before he/she resumes work at that facility.   Blackburn's Fabrication, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii)(C), which was contained in OSHA inspection number 191138, citation number 2, item number 1 and was affirmed as a final order on March 12, 2012, with respect to a workplace located at 2467 Jackson Pike, Columbus, OH 43223.
