29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Production - The employer did not ensure that lockout tagout devices were applied to isolate energy sources on Slitter 1 when employees were performing maintenance tasks. Employees could have been injured by the energized equipment while performing setting up procedures. (b) Production - The employer did not ensure that lockout tagout devices were applied to isolate energy sources on Slitter 2 when employees were performing maintenance tasks. Employees could have been injured by the energized equipment while performing setting up procedures. (c) Production - The employer did not ensure that lockout tagout devices were applied to isolate energy sources on Slitter 3 when employees were performing maintenance tasks. Employees could have been injured by the energized equipment while performing setting up procedures.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by in-going nip points and rotating parts; (a) Facility Wide - The employer did not ensure that guards or methods of guarding were permanently affixed to the machine to protect employees from rotating parts on Edger 1. (b) Facility Wide - The employer did not ensure that guards or methods of guarding were used to protect employees from rotating knives at the out-feed section on all slitters. (c) Facility Wide - The employer did not ensure that guards or methods of guarding were used to protect employees from in going nip points on Slitter 3 at the in-feed section.
