Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:    a) Production Area- On or about November 23, 2015, the employer failed to ensure the Keller 97 Series 6' Fiberglass Stepladder, Model 976, was in a safe condition prior to being used by employees.  The ladder had three bent safety shoes, two bent supports on the rung side and one bent horizontal support on the back side.  Additionally, the ladder was used on an uneven surface, which only allowed three of the four feet of the ladder to touch the ground at any given time.      No abatement certification or documentation is required for this item.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred.    a) 520 Santa Rosa Dr. Facility- The employer failed to log injuries within seven calendar days of the occurrence on the OSHA 300 Log, or equivalent form, during the calendar year 2015.  A work-related recordable injury that had occurred on or about November 23, 2015, was not entered on an OSHA 300 Log as of December 1, 2015.    No abatement certification or documentation is required for this item.
