Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:    a) Production Area- On or about November 23, 2015, the employer failed to ensure the Keller 97 Series 6' Fiberglass Stepladder, Model 976, was in a safe condition prior to being used by employees.  The ladder had three bent safety shoes, two bent supports on the rung side and one bent horizontal support on the back side.  Additionally, the ladder was used on an uneven surface, which only allowed three of the four feet of the ladder to touch the ground at any given time.      No abatement certification or documentation is required for this item.
