The power cord and strain relief were pulled away from the circular cold saw housing exposing live parts.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): At this fabrication facility, employees engaged in work activities, such as, but not limited to, steel fabrication and steel fabrication support activities, operated the two Hyster forklifts listed below with unsafe conditions: a. Hyster H100XL, serial number G005001910P, no back-up alarm, nonfunctional lights. b. Hyster 80, serial number not available, no back-up alarm, nonfunctional lights. Henderson Fabrication, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 316301910, citation number 01, item number 005, and was affirmed as a final order on October 9, 2012, with respect to a workplace located at 3107 Nichols Avenue, Bay City, Texas 77414.
