29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer did not ensure that hazardous moving parts on machinery were properly guarded, exposing employees to amputation hazards, in that: (a) The shear and press on the Piranha Iron Worker were not guarded; (b) The point of operation on the Piranha hydraulic press was not guarded, and; (c) The unused portion of the blade on the Hydmech horizontal band saw was not guarded.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) A power cord on the Controlled Automation CNC machine was not provided with strain relief and the outer sheathing was pulled away from the connector; exposing the inner wiring. Henderson Fabrication, Inc. was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 961336, citation number 1, item number 2 and was affirmed as a final order on May 28th, 2014, with respect to a workplace located at 3107 Nichols Avenue in Bay City, Texas 77414.
