29 CFR 1910.36(g)(2): An exit access was not at least 28 inches (71.1 cm) wide at all points and the width of the exit and exit discharge was not at least equal to the width of the exit access. a) Jeannie's Diner, Bldg 52010, Fort Huachuca, AZ 85613: On or about 5 March 2014 and at times prior thereto, egress from the kitchen was limited to 21 inches by the metal table and refrigerator/freezer unit.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: a) Jeannie's Diner, Bldg 52010, Fort Huachuca, AZ 85613: On or about 5 March 2014 and at times prior thereto, employees did not wear chemical splash goggles to protect them from severe eye injury. Lime-A-Way and Pure Bright Ultra Bleach, containing phosphoric acid and sodium hypochlorite respectively, is used to clean and disinfect kitchen surface areas.
