29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  (a) Mountain View Golf Course, Bldg. 15479, Wilcox Road, Fort Huachuca, AZ 85670:  On or about 6 March 2014, and at times prior thereto, the employer did not verify on a written document identified as such that a workplace hazard assessment had been conducted which nessitated the use of personal protective equipment by employees, exposing employees to hazards.

29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  (a) Maintenance Shop, Mt View Golf Course, Fort Huachuca, AZ 85760:  On or about 6 March 2014, and at times prior thereto, a spinning grinder used to sharpen mower blades was not anchored securely to the floor to prevent walking or moving while operating, exposing employees to hazards.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) Cart Shed, Mt View Golf Course, Fort Huachuca, AZ 85670:  On or about 6 March 2014, and at times prior thereto, two overcurrent devices in Panel A, switches # 26 (20 amp) and #28 (30 amp), were not listed in the panel legend as to location and purpose, exposing employees to hazards.

29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles:  (a) Maintenance Shop, Mt View Golf Course, Fort Huachuca, AZ 85670:  On or about 6 March 2014, and at times prior thereto, an extension cord plug did not have a grounding prong, peventing proper continuity of the grounding conductor and exposing employees to hazards.
