29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Sportsman's Center, Bldg. 15423, Fort Huachuca, AZ 85613:  On or about 6 March 2014, and at times prior thereto, the employer did not ensure that listed and labeled electrical equipment was plugged directly into 120v receptacles, in accordance with instructions in the labeling, in the following instances:  (1)    GE microwave, atop ltable in lounge, was plugged into an extension corde; (2)    Mr. Coffee coffeemaker, atop table in lounge, was plugged into an extension cord; (3)    Toaster, atop table in administrative area, was plugged into a power strip; (4)    Ms Tea teapot, atop table in administrative area, was plugged into a power strip,  exposing the employees to fire hazards.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) Electrical Panel A, Sportsman's Center, Bldg. 15423, Fort Huachua, AZ 85613: On or about 6 March 2014, and at times prior thereto, the employer did not ensure that circuit breaker switch #1 was legible and that blanks for slots # 25-28 were identified on the legend as to location and purpose, exposing employees to hazards.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  (a) Sportsman's Center, Bldg. 15423, Fort Huachuca, AZ 85613:  On or about 6 March 2014, and at times prior thereto, the employer did not ensure that all electrical outlet boxes installed in the back office had a cover or faceplate affixed, exposing the employees to fire hazards.
