29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On and before March 4, 2014, on the Skytrak Telehandler towveyor assembly line located at 600 East Chestnut, Rd., Orrville, OH 44667, the employer did not ensure that employees performing Skytrak assembly work were protected from crushing and caught between injuries when they were allowed to work on the assembly line that automatically cycled every 52 minutes with only an audible warning given 15 minutes prior to cycling and 15 seconds before it cycles.
