On or about March 6, 2014 at residential structure located at 218 W 6th St, North Little Rock, AR 72114, the employer did not ensure an employee framing a residential structure was properly protected by the use of a hardhat from impact hazards from construction materials . The employee was observed/exposed to struck by hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about March 6, 2014 at a residential structure located at 218 W. 6th street, North little Rock, AR 72114, the employer did not ensure an employee framing a residential structure was properly protected by the use of safety glasses while using the pneumatic nail gun.  The employee was observed exposed to eye impalement hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about March 6, 2014 at a residential new home construction structure located at 218 w. 6th street, North Little rock, AR 72114, the employer did not ensure workers had a safe access to the roof where they were assigned to work.  Employees used an 7 foot self supporting ladder to access the roof of the structure.  The condition exposed employees of falls greater then 10 feet to the finished ground below.
