29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. At the jobsite where damaged extension cords were being used, an inspection of the cords was not completed prior to use, thereby exposing the employees to a contact hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. Between buildings A and B where an extension cord was ran across a roadway, the cord was not protected from damage due to being ran over and had already sustained damage to the cord, thereby exposing employees to a contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. At building A where employees used extension cords to power a circular saw, the strain relief clamp was pulled away from the outer insulation jacket of an extension cord, thereby exposing the employees to a contact hazard.
