29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees working drilling rig #6, located at SEC 31, TWP 14, R 31, 1370 fnl, 1130 fsl, Oakley, KS were exposed to fall hazards of greater than four feet while walking and working in the following locations: a) Generator platform; a 73 inch section did not have top rails installed, distance from walking surface to the ground was 59 inches. b) Drilling floor; outside of dog house; there was a 13 inch gap in the guard rail between two platforms, distance from working surface to the ground was 6 feet, 10 inches.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. Employees working drilling rig #6, at SEC 31, TWP 14, R 31, 1370 fnl, 1130 fsl, Oakley, KS were exposed to caught in and struck by hazards in that the pony rod of the Skytop Brewster high pressure mud pump was not fully guarded. There were openings of 12 inches x 25 inches on both sides of the metal housing.
