29 CFR 1910.95(g)(7)(ii): Where the annual audiogram showed that an employee had suffered a standard threshold shift, the employer did not obtain a retest within 30 days.    (a) Facility: On or about March 10, 2014, the employer failed to retest an employee that had suffered a standard threshold shift.    On April 11, 2014, employees working in the testing department located inside of building 110 were exposed to continuous noise at a level of 332.3% of the permissible exposure limit dose. The equivalent dBa for 332.3% of the dose is approximately 98.6 dBa calculated as the 8 hour time weighted average sound pressure level. The monitoring was conducted for 402 minutes. Zero exposure was assumed for the remaining 78 minutes of the day.

29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary:    (a) Facility - On or about March 10, 2014, an employee who suffered a standard threshold shift, was not refitted and retrained in the proper use of hearing protectors. A physician had not determined that the threshold shift was not work-related.    On April 11, 2014, employees working in the testing department located inside of building 110 were exposed to continuous noise at a level of 332.3% of the permissible exposure limit dose. The equivalent dBa for 332.3% of the dose is approximately 98.6 dBa calculated as the 8 hour time weighted average sound pressure level. The monitoring was conducted for 402 minutes. Zero exposure was assumed for the remaining 78 minutes of the day.

29 CFR 1910.95(j)(1): The employer did not evaluate hearing protector attenuation for the specific noise environments in which the protector will be used and use one of the evaluation methods described in Appendix B: Methods for Estimating the Adequacy of Hearing Protection Attenuation:  (a) Building 110 (Outdoor Testing Area) - On or about May 13, 2014, the employer failed to select and require the use of hearing protectors that reduced employee noise exposure to below the permissible exposure limit of 90dB over an 8 hour period.   On April 11, 2014, an employee working in the testing department located outside of building 110 was exposed to continuous noise at a level of 1266.0% of the permissible exposure limit dose. The equivalent dBa for 1266.0% of the dose is approximately 108.3 dBa calculated as the 8 hour time weighted average sound pressure level. The monitoring was conducted for 395 minutes. Zero exposure was assumed for the remaining 85 minutes of the day.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    (a) Building 110 - Interior Testing Area: On or about March 10, 2014 6 compressed gas cylinders, containing tetrafluoroethane, were not stored securely to prevent them from falling. 29 CFR 1904.10(a): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log.  (a) Facility - On or about March 10, 2014, and at times prior to, the employer failed to record a standard threshold shift that averaged greater than 10 decibels with a total hearing level of 25 decibels or greater.
