29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews. (a) Front of Building - On or about March 10, 2014 an employee was exposed to a fall hazard while working on a scaffold 21.3 feet above concrete. Sections of guardrail system was missing on the ends of platform. Ropes were used in sections of guardrail system.

Scaffolds shall be plumb, level, and squared. All brace connections were not secured. (a) Front of Building - On or about March 10, 2014 an employee was exposed to a fall hazard due to a scaffold having a ladder that did not reach within 2 feet of the ground. The access point was 7.6 feet above concrete.
