29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews.     (a)  Front of Building - On or about March 10, 2014 an employee was exposed to a fall hazard while working on a scaffold 21.3 feet above concrete. Sections of guardrail system was missing on the ends of platform. Ropes were used in sections of guardrail system.

29 CFR 1926.452(w)(1): Scaffolds were not braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections were not secured.    (a)  Front of the building - On or about March 10, 2014 an employee was exposed to a fall hazard while working on a scaffold that was missing cross-braces.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   (a)  Front of Building - On or about March 10, 2014 an employee was exposed to a fall hazard due to a scaffold having a ladder that did not reach within 2 feet of the ground.  The access point was 7.6 feet above concrete.

Scaffold had incomplete guardrails, no ladder and missing cross-braces.

The training shall include the following areas, as applicable:   (a)  Construction Site - On or about March 10, 2014 employees were not provided training on how to recognize, prevent and minimize fall hazards associated scaffolds.
