(a) Block 4 - On or about January 21, 2015 an employee was exposed to a flying object hazard when drilling into an asphalt roadway without safety glasses.

29 CFR 1926.651(d): Exposure to vehicular traffic. Employees exposed to public vehicular traffic shall be provided with, and shall wear, warning vests or other suitable garments marked with or made of reflectorized or high-visibility material. (a) Block 4 - On or about January 21, 2015 four employees were exposed to a struck-by hazard from automobiles when working in roadway without wearing high visibility clothing.
