29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) The inspection revealed that employees were potentially exposed to serious injury from the use of the Edwards 100 ton ironworker without having the guards in place on components of the machine that were not in use. The ironworker was located inside the shop at Hangar 3, North Tongue Point in Astoria, Oregon.

29 CFR 1910.1200(f)(6) is applicable to shipyard employment by 29 CFR 1915.1200 that incorporates the General Industry requirement: The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party.  (a) On or about March 6, 2014 the inspection revealed a spray bottle that was not marked to indicate the contents or other required information. The spray bottle was located inside the flammable storage shipping container in on the dock yard at Hangar Way in Tongue Point, Astoria, Oregon.   (b) On or about March 6, 2014 the inspection revealed a parts washer that with an unidentified liquid chemical inside. The parts washer was located in the north side of the shop at #3 Hangar Way at Tongue Point, Astoria, Oregon.

29 CFR 1910.1200(j)(1): 29 CFR 1910.1200(j)(1):  The employer did not train employees regarding the new label elements and safety data sheets format by December 1,2013.   (a) On or about March 6, 2014 the inspection revealed that employees had not been given training on the new hazard communication label elements and safety data sheet (SDS) format that was required to be conducted by December 1, 2013. Located throughout the facility at #3 Hangar Way, Tongue Point, Astoria, Oregon.
