29 CFR 1910.177(d)(4)(i): The employer did not furnish and assure that an air line assembly was consisted of a clip-on chuck for inflating tires on rim wheels: Tire Changing Area: Employees were exposed to struck by hazards due to the air line assembly was not consisted of a clip-on chuck that would enable the employees to stand away from the tire filling process. On or about March 12, 2014, a single piece rim wheel/tire burst and landed with the safety cage on the employee's leg while servicing the wheel/tire, and the employee sustained a contusion to the leg.

29 CFR 1910.177(d)(4)(iii): The employer did not furnish and assure that an air line assembly was consisted of a sufficient length of hose between the clip-on chuck and the in-line valve which allowed the employee to stand outside the trajectory while inflating tires on rim wheels: Tire Changing Area: Employees were unable to stay out of the trajectory and exposed to struck by hazards due to the air line assembly consisted of a insufficient length of hose between the clip-on chuck and the in line valve. On or about March 12, 2014, a single piece rim wheel/tire burst and landed with the safety cage on the employee's leg while servicing the wheel/tire, and the employee sustained a contusion to the leg.
