(b) Roll Ring Area: A bandsaw had 20 inches of unused blade that was not guarded, as determined on or about March 11, 2014. (c) Shop Area: A Warner and Swasey Lathe, Serial No: 774975 was not provided with a chuck guard, as determined on or about March 11, 2014. (d) Shop Area: The unused portion of the belt was not guarded on the Porter Cable Machine Company, vertical belt sander, type G-8, as determined on or about March 11, 2014. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Shop Area: Employees perform maintenance and service on machinery such as, but not limited to the 175 ton Cincinnati brake press, serial # 42743, with electrical and hydraulic power sources which requires the control of hazardous energy by locking out the equipment. The employer has not conducted an annual inspection of the energy control procedures, as determined on March 11, 2014. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM Danser, Inc. was previously cited for a violation of this occupational safety and health standard, The control of hazardous energy (lockout/tagout), which was contained in OSHA inspection number 313377137, citation number 2, item number 1 and was affirmed as a final order on November 30, 2010 with respect to a workplace located at Danser, Inc. located in Parkersburg, WV.
