29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Head Saw: The employer did not utilize the written Lockout/Tagout procedure when performing service or maintenance on the Simmons head saw. Prior to sharpening the 56 inch single circular blade, a lock is not placed on the disconnect switch in accordance with the written procedure developed by Pierson Lumber Company, Inc., as determined on March 3, 2014. ABATEMENT IS REQUIRED FOR THIS ITEM.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: (a) Sawmill: Employees perform maintenance and service on machinery such as the Simmons head Saw, with electrical and mechanical power sources, which requires the control of hazardous energy by locking out the equipment. The employer did not certify that the annual inspection of the energy control procedures had been conducted, as determined on March 12, 2014. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM ABATEMENT NOTE: The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.
