29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared: (a) Mill: On or about March 10, 2020 the employer had not established a valid baseline audiogram for each employee. An employee (Debarker Operator) was exposed to continuous noise at 70.8 % of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 459 minute sampling period on March 10, 2020. For a full eight hour work shift this exposure would be equivalent to an average sound level of 87.5 dBA. Exposure calculations included a zero increment for the 21 minutes not sampled.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : (a) Mill: On or about February 25, 2020 the employer had not obtained a new audiogram at least annually for each employee. An employee (Forester/Lumber Grader) was exposed to continuous noise at 98.2 % of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 469 minute sampling period on March 10, 2020. For a full eight hour work shift this exposure would be equivalent to an average sound level of 89.8 dBA. Exposure calculations included a zero increment for the 11 minutes not sampled. (b) Mill: On or about February 25, 2020 the employer had not obtained a new audiogram at least annually for each employee. An employee (Trim Saw Operator) was exposed to continuous noise at 177.5 % of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 468 minute sampling period on March 10, 2020. For a full eight hour work shift this exposure would be equivalent to an average sound level of 94.1 dBA. Exposure calculations included a zero increment for the 12 minutes not sampled. (c) Mill: On or about February 25, 2020 the employer had not obtained a new audiogram at least annually for each employee. An employee (Edger Saw Operator) was exposed to continuous noise at 260.7 % of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 478 minute sampling period on March 10, 2020.
