29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  On or about 3rd Floor, North Side  a) The wheel locks had wire around them to keep them in the open position. The employee was working on the scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers:    On or about 3/12/2014, 3rd Floor, North Side    a) The scaffold was at the edge, while the employee was spraying monokote on the beams. He was exposed to an approximate 40' fall hazard to the outside of the building.
