29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    Prudential Center buildings:  The employer did not provide appropriate work uniforms to the electricians in that the uniform content of the shirts was 60 percent cotton and meltable fibers of 40 percent polyester.  Additionally, some electricians were provided with short sleeved uniforms.  The uniform pants provided were 100 percent cotton which does not provide protection from arc flash hazards.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    Prudential Center buildings:     (a)  Employees who perform electrical work and testing on energized electrical equipment and systems were not effectively trained in implementing electrical safety-related work practices for tasks such as changing lighting ballasts, removal and installation of overcurrent devices, and troubleshooting and repair of electrical equipment of up to 480 Volts.    (b)  Employees had not been effectively trained in the selection and use of electrical personal protective equipment such as uniform clothing selection, arc flash suits, head protection, and electrical rated gloves.

29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them:   (a) 111 Huntington Ave, Boston - 26th Floor:  An employee was shocked and burned when contact was made with live parts while testing and changing a lighting ballast where the circuit had not been de-energized.  (b) Prudential Center buildings:  Employees were exposed to live electrical parts while performing tasks including but not limited to changing ballasts for 277V lighting fixtures and installing and replacing overcurrent devices in equipment of up to 480 volts where the circuits had not been deenergized.
