29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) 3rd Shift Sanitation: On or about March 14, 2014 and at times prior thereto, the employer did not provide and require the use of chemical goggles or face shields while conducting sanitation related activities (e.g. mixing, application, and rinsing) of Al Chlor, Zo Chlor, and Zantize which exposed employees to severe irritation and burns related to exposures to Sodium Hypochlorite and Sodium Hydroxide.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  a) Hazard Communication: On or about March 14, 2014 and at times prior thereto, the employer did not provide effective training to enable the employee to recognize the hazards and safety precautions (eye protection) while working with and/or around  Al Chlor, Zo Chlor, and Zantize which exposed employees to severe irritation and burns related to exposures to Sodium Hypochlorite and Sodium Hydroxide.

29 CFR 1910.303(b)(6): Conductors or equipment not identified for use in a damp or wet location were used:  a) North Wall of the Shipping Area: On or about March 12, 2014, the employer did not ensure a 4 receptacle outlet had ground fault circuit interrupter (GFCI) protection which exposed employees to electric shock hazards.  b) South Wall of the IQF Area: On or about March 12, 2014, the employer did not ensure a 4 receptacle outlet had ground fault circuit interrupter (GFCI) protection which exposed employees to electric shock hazards.  c) West Wall of the IQF Area: On or about March 12, 2014, the employer did not ensure a 2 receptacle outlet in series had ground fault circuit interrupter (GFCI) protection which exposed employees to electric shock hazards.  d) Trim Station of the Kill Area: On or about March 12, 2014, the employer did not ensure four 2 receptacle outlets in series had ground fault circuit interrupter (GFCI) protection which exposed employees to electric shock hazards.
