29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection:    a)  At Ice House, Southfresh Aquaculture, Eutaw, Alabama:  On or about May 30, 2017 and at times prior; the employer exposed employees to caught-by hazards in that employees were allowed to place part of his/her body above the rake portion of the machine without first locking out the machine to prevent accidental start-up.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a)  At Maintenance Shop, Southfresh Aquaculture, Eutaw, Alabama:  On or about June 5, 2017 and at times prior; the employer exposed his employees to electrical hazards in that employees were allowed to work around open breaker slots exposing live 480 volt buss bars inside the  HMP-C electrical panel.
