29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.       a. On or about March 18, 2014, at the above located workplace, employees required to perform plating operations were exposed to hazards associated with chemicals. These hazards necessitated the use of personal protective equipment such as safety glasses and gloves to prevent occupational illnesses. The employer had not verified, through written certification, that a hazard assessment had been performed.

29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following:      (i) When personal protective equipment is necessary; (ii) What personal protective equipment is necessary; (iii) How to properly don, doff, adjust, and wear personal protective equipment; (iv) The limitations of the personal protective equipment; and (v) The proper care, maintenance, useful life and disposal of the personal protective equipment.  a. On or about March 18, 2014, at the above located workplace, employees required to perform plating operations were exposed to hazards associated with chemicals. These hazards necessitated the use of personal protective equipment such as safety glasses and gloves to prevent occupational illnesses. This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section:      a. On or about March 18, 2014, at the above located workplace, the employer did not determine the 8-hour TWA exposure to chromium (VI) for employees required to perform plating operations.
